Lice/mites can kill your chicken she needs treatment fast dust with sevins or poultry dust. Clean and treat coop too. Repeat in ten days. Wood ash is great for preventing them in the future but she needs help now.

If she does not start drinking on her own she will die if you don't get her re-hydrated. Can you take her to a vet? If not, you need to read these:
