Hi! My polish chicken today is lethargic. I noticed her acting different over the past week and started giving her vitamins and vitamin E. No swollen crop. She is vaccinated. I just tried an epsom salt bath and no improvement. I’m a first time chicken owner so any advice would be appreciated.

That's pretty severe lethargy.
If she was young and otherwise healthy until recently and she passes, you should have a necroscopy done to determine if it's something contagious.
Are any of the others acting off?

No all others are acting normal
 
I would remove her from the flock to isolate her. Put her in a warm location such as a dog crate or in a bathroom on a non skid surface, where you can inspect her and monitor her. Have you checked her for wounds or injuries? Is she not able to stand or walk? Not sure what we're looking at in the first photo?

what was different? Was she pooping, eating, drinking normally until today?

I would not bathe her anymore as this can be very stressful for chickens and may send her over the edge.

How old is she and when did she lay her last egg?

How old is she?
What do you feed, including treats?

What vitamins are you giving?

I wouldn't give her another bath, this can be stressful and if a bird is in crisis it can push them over the edge.
You may want to however, provide her with some supplemental heat, keeping her at around 70-75F. Making sure if you use a heat lamp or place her on heating pad, she's able to move off or away from the heat source.

Have you treated for Coccidiosis?
Corid can be found at TSC.
Liquid Corid (Amprolium) dose is 2tsp or Powdered Corid Dose is 1 1/2tsp per gallon of water given for 5-7 days as the only source of drinking water.
Do not add any extra vitamins/electrolytes that contain B1(Thiamine) to food or water during the course of treatment.
