Lethargic Hen

I have a small flock of four hens that I recently purchased as pullets. Last night my Rhode Island Red did not roost and slept hunkered on the coop floor. Today she is very lethargic and won't come out of her run even after I nudged her repeatedly. She just started laying so she is still young. I feed them a mix of scratch and lay pellets and I have never had a problem with food before. What might be wrong with her and how could I treat it?
 
Update: she has begun to throw up clear liquid. She is puffed up and her poop is liquid and white. There is a broken egg in the nest but we don't know if it is hers. Could she be egg bound? We have given her a mineral salt bath to see if that helps.
