Mini hen laid only yolk?!

It looks like a shell-less egg, but is it soft or firm? The diet you are feeding does not sound very good for egg laying. Is that scratch grains mostly? Do you feed a balanced chicken layer or all flock feed? Do you have crushed oyster shell available? I would make sure that she is getting a balanced chicken feed as most of her diet. Limit treats. For a few days I would offer a human calcium tablet with D3 to see if her shells become hard. I have had hens occasionally lay a tiny egg. If increasing calcium and a balanced diet don’t help, then she might be suffering from salpingitis (inflammation of the oviduct) or a defective shell gland.

Do you think it could be the disease I mentioned?

Egg yolk peritonitis is infection inside the abdomen with masses of internally laid egg material and pus. It may be the late stage of several reproductive problems. What I see in the picture is just a shell-less egg, which can be due to low calcium or a shell gland problem. In this article read number 10 which has possible causes of shell-less eggs:

She eats a mix of scratch mix and fresh veggies. The treats include kale, carrots, apple, yogurt drinks, and meal worms. We also give her the durastat oregano powder in the water. She has been sick 2 times before tho it wasn’t diagnosed what it was. Both times included her being very gurgly and heavy breathing but both times they went away on their own before we could get her to the vet. When she stands her tail is aimed down but she is standing a good bit more than earlier this morning. But she still isn’t fully acting like herself. I included a picture of the yolk in question. My mom believes it is just a yolk but it reminds me more of a regular soft shell egg. I’m too paranoid to open the yolk incase I need it when I do get the chance to bring her to the vet.
That isn't a great diet which is probably not helping the soft shelled eggs. Scratch is not feed, it's a treat, think of it like cheetos. Fruits and veggies and meal worms, and yogurt are fine but should not make up more than 10% of the diet, she needs real formulated chicken feed, like All Flock or Flock Raiser and oyster shell or egg shells on the side. It does sound like a soft shelled egg broke in her or was just a soft shelled egg. if the egg membrane is still in her, it will likely come out on its own but you might need to help her.

Sorry, i have used it for my chickens though and it is helpful. But you are a pro so you should probably be the one talking to the starter of the thread.
Oh no, I'm sorry, I didn't intend to come off like a pro, I'm still learning. The salve is basically a chapstick, it just protects the skin if there's cuts or scrapes, that's all. it's not going to help in this instance.

Thank you for the info. I’m still extremely new to taking care of chickens (my neighbor dumped roughly 8 chickens on me less than a year ago and I’ve been trying to get them healthy again) I’m gonna see about swapping her diet. Hopefully the feed store will have one of the ones you mentioned. She also has unlimited access to oyster shell and grit. Her diet basically is: a bowl of scratch mix with another bowl of oyster shell and she gets fed the veggies and other treats either in the morning or around noon every other day. But I will absolutely swap her scratch mix for better feed. I’ve only ever owned ducks so it’s a process to learn the difference in care
