Missing egg or chick

My hen was sitting on 12 eggs. 8 of them hatched out with no problem, one died halfway out of the egg, two eggs are still under the hen, and one is just missing altogether. What could cause one to go missing? The hen is in a large tub upstairs in my house. It is covered so nothing can get in or out... I mean, it just vanished into thin air

This is one of my biggest questions too. I have 15 eggs and 8 goes missing. Thank you for your post I want to know the answer too
