Yes, it has to go on the chickens, otherwise the mites that are on them are not going to go away. Generally if one bird has mites they are all likely to have them to some extent. If you don't treat them all and the untreated bird has some they will just repopulate. They can be hard to get rid of that's why you have to retreat and use something effective.
As far as how do you know if you've cleared them up? You just have to keep checking them, check the roosts in the coop at night when mites like to come out to feed and check the birds carefully. And there is no egg withdrawal on birds that have been dusted.
