my chicken is sick

I have a polish hen . She is a one year old. Recently I noticed he laying on her side . But when she's not laying down, she is normal. I also noticed her snorting, that sounded like a pig. When we picked he up her nose was covered in a dirt like snot . I have no clue what was wrong with her. Now my silkie hen has dirty nostrils. I'm really worried my newly hatched chicks will get it as well. Is there something I can do to cure it ? What's even wrong with them?
