What causes it and what to do about it?

I usually start by de-worming with Safeguard liquid or paste at 0.23ml per pound. Are you seeing a lot of poop like this or just from one bird?

I just found several poops like this in my coop, maybe even more watery though. The hen I found limping yesterday appears to have some staining on herr feathers around her vent so I assume it is from her. Could this be the result of an injury? I have heard some coughing/sneezing from a few birds but assumed it was just a cold as there are no other symptoms and no nasal discharge. What should I do?
