Old hen

Hi I have two red hens.one is an ex battery wihich I've had couple of years. She is looking pretty rough so I've been giving her the apple tonic but not making any difference.her head has been going bald for a while, her feathers are turning white and falling out and she has a crusty foot. What should I do with her?
