Older hen 4 years old having issues?

I have a buff orphington hen that can't move very well and she looks swollen all over. She is in a flock of 21 other hens she has good looking feathers but is going down hill every day. My wife cleaned her vent it was somewhat plugged with manure and the wife cleaned her good with warm water but I notice her health getting worse every day. I am guessing her age at 4 years old the reason I don't know is I was given one buff and I had 2 already so I am not sure of the age of one of them I have had her for years though. Is there something I should look for? I keep expecting her to be dead at the rate she is going. Can they get an egg plugging them up?

Elderly hens can be more likely to have reproductive tract problems. This may very well be what she is dealing with.
