One Poo Butt Hen

I have six two year old hens.
My silver laced wyandotte has always been on the small side but she is very strong and seems healthy beside the poo.

Her butt is always leaking a chalky gritty, crusty, watery poo.
Her poor butt is always red where she seems to be trying to pull out her poo feathers.
She never has a fluffy butt like the others...

Things I've tried to help her unsuccessfully:

Bath 2 times per week.
ACV in water. Very clean waterer.
Powdered probiotics mixed in feed.

I did these things for about a month with no improvement.
Any other ideas of things I can try to help her out?
 
Maybe clip the feathers around her vent? We had a bird that we think got sick (don't know how or with what, but she suddenly went down hill really fast before she seemed to start doing better) who had bad poop. Once we clipped the feathers, there wasn't much for it to get stuck on and she didn't have to deal with it getting dry and heavy
