Ontario Canada - Form 300 Small Flock Grower - Required to send to processing?

I have a couple young roosters that will need to be processed later this fall but I've been researching and all of the processing plants in Ontario require a "Form 300 Small Flock Grower" form filled in. The form should have been given to me when I purchased my chicks from the hatchery......except that I purchased my birds from a neighbour, who purchased from a neighbour, etc.

Anybody dealt with this before? Is it worth "trying" to register...to process 3 birds?
I'm pretty sure my weak stomach wont let me do it myself either!
