Our duck is 1 month old but still small

To even get close to a idea of what might be going on with a bird it is vital you provide thorough, and detailed information about the bird's past history, current condition, and environmental conditions in which it is living.
Without, help becomes pretty useless.

Example:
1) What type of bird , age and weight (does the duck seem or feel lighter or thinner than the others.)
2) What is the behavior, exactly.
3) How long has the bird been exhibiting symptoms?
4) Are other birds exhibiting the same symptoms?
5) Is there any bleeding, injury, broken bones or other sign of trauma.
6) What happened, if anything that you know of, that may have caused the situation.
7) What has the bird been eating and drinking, if at all.
8) How does the poop look? Normal? Bloody? Runny? etc.
9) What has been the treatment you have administered so far?
10 ) What is your intent as far as treatment? For example, do you want to treat completely yourself, or do you need help in stabilizing the bird til you can get to a vet?
11) If you have a picture of the wound or condition, please post it. It may help.
12) Describe the housing/bedding in use

The duck ate alot of watermelon then it started to be like this all of the sudden we tried to feed it other things it didnt eat then it drank water with lemon and onion and it ate some onion but its not good still

Is your duck on actual waterfowl grower feed? Also please stop right now with the lemon/onion in the water. Only clean fresh water and on the side you can provide another dish with a poultry vitamin/mineral/electrolyte supplement.

Onion is one of those did you do not want feed ducks. A little bit every once in a while is not bad, but it can become deadly with too much. How old is your duck?

It sounds like you may not be providing the duck any formulated feed. Is this true? Please tell us everything you have been feeding him this week? Do you provide grit?

Is there a chance you get in contact with an experienced poultry keeper in your area so they can look over your setup, and help give some pointers?
Pardon my bluntness, but you are obviously undereducated on caring for ducklings if you are trying to offer lemon and onions in a dish; both of which will negatively affect the health of the bird.

I suspect the issues being seen are due to improper management. Please read up on proper duckling care:

wasnt my idea i asked someone on discord and he said to give lemon and onion

I see you are using newspaper for bedding? I recommend not using newspaper as it can cause leg problems from it being slippery on their feet. You could use wood chips instead. I also recommend giving him a friend.
