4) Are other birds exhibiting the same symptoms?
5) Is there any bleeding, injury, broken bones or other sign of trauma.
6) What happened, if anything that you know of, that may have caused the situation.
7) What has the bird been eating and drinking, if at all.
8) How does the poop look? Normal? Bloody? Runny? etc.
9) What has been the treatment you have administered so far?
10 ) What is your intent as far as treatment? For example, do you want to treat completely yourself, or do you need help in stabilizing the bird til you can get to a vet?
11) If you have a picture of the wound or condition, please post it. It may help.

Is your duck on actual waterfowl grower feed? Also please stop right now with the lemon/onion in the water. Onion is one of those did you do not want feed ducks. A little bit every once in a while is not bad, but it can become deadly with too much. Pardon my bluntness, but you are obviously undereducated on caring for ducklings if you are trying to offer lemon and onions in a dish; both of which will negatively affect the health of the bird.
I suspect the issues being seen are due to improper management. Please read up on proper duckling care:

Do you have a photo? What type of feed is the duck on? Do you have other ducks?
I see you are using newspaper for bedding? I recommend not using newspaper as it can cause leg problems from it being slippery on their feet. You could use wood chips instead. I also recommend giving him a friend.
