Pipped, but died in shell (cw dead chick pictures)

I had a pretty successful hatch - 17/24 - but I lost two within the past few days, and I’m wondering if it’s anything I could have prevented so I might know better for next time.
The first one never internally pipped and hadn’t absorbed most of the yolk. This one did pip, but never made it out. :(

Did I shrink wrap it? Can you even tell? It didn’t look like the membrane was stuck, but…I did the incubator to grab the 17 chicks this morning because they were overcrowded and squishing each other. Even then, I don’t think the humidity dropped below 65%, but…

I’m sorry, I should have taken pics before I opened the shell. I wasn’t thinking.

That does make me feel better. I had 23/24 hatch a month ago, but even though I know it’s not realistic to keep those numbers, (especially since these were shipped eggs) I was still concerned it was something I did this time.
I kept 99.6 and 45% until day 18; 65% for lockdown. (Post hatch it spiked to 85 or so from all those wet chicks :))
 
like said, sometimes they die .. ive done it before though, i dont believe in lockdown as a rule 'but', a drastic change in conditions right at hatch time can cause shock .. its not 'just' humidity or any other single thing .. if the bator has the lid lifted off for several minutes though, or eggs are shifted say from an upright to side position, moved to another location etc etc, it can throw them off and shock them .. like i said, ive done it and seen it happen .. i grab chicks out as they hatch but try to make it quick, and definitely, definitely now is not the time to play around with pipped eggs candling them etc ... 'turn off air', tilt lid, grab chick and maybe shell, turn any down pips up and close it, id say under 30 seconds .. let it restabilize a good while before opening again for anything ..

Humidity is good but think I've read lockdown 70% ... What needs to be taken in consideration it the ambient temp/humidty. There are times "things" happen and we have no clue why. A friend who has had no issues with incubation has had 3 sets fail this year ... Some just stop developing and there has been a few like yours; fully formed and pipped but not make it out. I have a singleton from her last attempt that I'm raising due to CTP which I've treated. We tried purchasing couple more chicks for company but never was able to find the right age ... She's now 6wks and ready to go back but not until she's bigger to join the flock. I got the room so no problem, she's a sweet heart too.

We do the best we can and Nature takes it's course.
