could be pecking , overly aggressive rooster that wanted to mate and she didn't or scratching. First thing to do is check the hen for external parasites like lice/mites and treat of need be. As we don't know your flock make up or set up kinda hard to give advice.

Chickens will peck at bugs on another chicken, out of boredom, or their version of pms when molting or even being over crowded. A rooster can do this kind of damage mating also. do you happen to know this chickens position in the pecking order?

Without more information and perhaps another picture, hard to tell if it pecking or some other injury, possible self inflicted from scratching. If more scabs are seen, might be pox, but cannot say at this time. Is this chicken being bullied by another? Some observation during the day and at roost time might be needed.
