Problem with large egg

I got an unusually large egg from one of my birds. When I opened it, I found there was another egg inside with its own shell. How could such a thing happen? Any reason for concern?

One of my quail laid an egg like that once. The outer egg was as big as a Silkie egg.

I don't know how she got it out, but she was fine afterwards and laid normally.

No, she's been laying since December. I've seen no other abnormalities other than that one egg, which was about five days ago.
