I have not. I don't know if our local vets treat chickens. If any do, I'd like to have one treat Rojo for his chronic bumblefoot, which I have given up on treating, since it's clean and dry and does not seem to bother him. I'd love for him to have an antibiotic shot for it. Other than that, no. I treat my chickens as advised by the Educators here on BYC.

Wow, I'm an outlier. Yes. I have a chicken vet less than 5 minutes from me.

I think there is a vet within an hour or so that would treat chickens. I've never taken one in, and I doubt I ever will.

No, it would depend on the situation.
For the sake of getting the chicken to live? Probably no, I treat at home however I can. If I suspected or knew my entire flock was at risk of something serious? Then potentially. Vets have better access to things such as diagnostic equipment, most of my vets do not enjoy, or are trained in, treating birds though.
