Do you mean that his foot was swollen?
It appears that he has lost the end of his middle toe to some type of injury. If it was very swollen then it may have been infected. I can't tell in the picture. I would soak the foot in warm water with some chlorhexidine (hibiclens), clean it as well as you can, apply some plain neosporin or plain triple antibiotic ointment. I would keep him on clean towels or puppy pads until it's healing up well. If there is any discharge from the area, redness, or bad smell, then an antibiotic may be needed.
