Rough shelled eggs and no shelled eggs

I have one Buff Orpington that lays eggs with either a really rough, thin shell or no shell at all. She has done this since she started laying a few months ago. At first they just had big bumpy spots on them and its progressed to being more and more without a shell. I have 23 hens and she is the only one that does this. I feed Layena pellets with free choice calcium and grit. Also give garlic and acv alternating in their water and give yogurt quite regularly for their good bacteria...am I missing something or does she just have something wrong with her that I can't do anything to help.
 
The Poultry Site and Alltech list various reasons for them , but if she is the only hen who seems to have a problem, and has been laying abnormal eggs all along, she may have a shell gland defect... or or some defect specific to her that for some reason she is not able to utilize calcium et correctly in making egg shells. It doesn't sound like there is much that can be done for shell gland defects. If you have more than one hen doing it, I would look into other causes, diet, disease etc.
