Shell- less eggs

I could really use some help please. I have some RIR's that are a year old now. They have fairly consistently had the same feed throughout the year, that consists of grains, vitamins grit and calcium all formulated in it. All of the other chickens have done well on it, but for the past couple of months we have been getting quite consistently a shell- less egg or a very soft egg with cracks that all come together that if you press in the center gently it will break. All of the other eggs are perfectly formed and consistent. I would love to hear what others more experienced than I have to say to help resolve this please. Is it an illness? A problem absorbing the calcium or ??? I would appreciate all the help I can get on this.
 
Do you have a link or photo of the feed you're using?

x2 on having additional calcium on the side, as some birds need more calcium than others. Layer feed is formulated to meet an average need.
