Sick Chicken

I have twelve chickens all of them are four months old, Noticed one of them a black jersey giant hen, acts weak, not eating much, does not move much not like usual, and has yellow diarrhea. I separated her from the rest of the flock in a small cage. She is drinking water, and eating a little. I started the whole flock on medicated chick starter, and switched them to flock raiser at 12 or so weeks old. Any advice to protect my flock and help the sick hen. This is my first flock.

I'm sorry to hear about your chicken! :( I had the same problem with one of my chickens with the same symptoms. Continue keeping her away from the other chickens and make sure to take her to the vet to get her checked out. The vet will probably give you antibiotics to feed her which help ALOT. Follow the vet's instructions and keep her away from the other chickens until she is in good shape.
If after giving her the antibiotics for the given time, and she is not getting better (which I doubt) take her to the vet again and carefully follow him/her instructions. I hope this helps! Good luck!
