Sick hen - eyes closed, lathargic, green mucousy poop - Help!

A few months over a year in age, she's lethargic, just laying down with her eyes closed - she opens them when I pick her up.
She had green mucousy poop this morning and I found a soft-shelled egg in the coop (can't verify it was hers, but seems plausible, as the others are healthy).

Any ideas as to what's wrong with her? What should I do?

I looked through the sick hen stickies, but didn't find what I was looking for - not to mention I have 2 youngs boys and a toddler pulling on me...any help would be so greatly appreciated!
