Sick hen - please help!

I have a hen who looks really bloated, her comb is almost a grey color, she's staying clear of the rest of the flock, and has lost her feathers around her vent/stomach.

Most concerning however - she has a green/yellow crust/slime covering her rear/stomach.

Any idea what this might be? Or how to help her out? I had a hen die from a similar condition a few weeks ago. :(

It could be that she has ascites or a water belly from internal laying/egg yolk peritonitis. Sometimes withdrawing fluid with a needle and syringe is done periodically to relieve pain and pressure, but unfortunately most hens will die from this. Another possibility is vent gleet.
