Sick turkey hen

I have a turkey hen who randomly started acting very sick this morning. She is lethargic and will be puffed up. She also hangs her head most of the time. The rest she is opening her beak and shaking her head.
Is this gapeworm?

She has a grape sized solid lump right under my middle finger

My other female turkey was attacking her so I had to separate them. I cant really find any reasoning for this sudden onset so any advice is appreciated.
This year is just too much
