Sleepy and falling down hen

I have a road island red hen that has seemed sleepy the last couple of days and I noticed she fell off her two feet and rolled on her back. I had to lift her up to get her back on her feet. I have her separated now and I will be feeding her some garlic water and spinach in her food. I will purchase vitamins today. Any ideas on the falling down part?
 
Welcome to BYC. You didnt state her age. Anyway, visually inspect her for lice/mites, especially around the vent area. She might be eggbound or egg impacted. When was the last time she laid an egg? Lastly, she may need to be wormed; or any combination of these things I mentioned.

Thank you for your quick response! She is about a year old at the most. I checked for mites and lice. I didn't see any and I couldn't tell if she was egg bound. The whole time I was checking her out she could barely keep her eyes open. She would not take any water or food either. We have had chickens for over 9 years and never experienced these symptoms. I will be taking her to the vet today.
