Sour crop

That could be part of the problem. A pea size amount *might* treat large roundworms, but it will not treat other worms, especially capillary worms. One pea sized dollop has only 25 mg fenbendazole in it, so it's like giving 1/4 ml of the liquid, which is not enough for a 5-pound bird. :(

She’s about 3 lbs right now— should I give her another round?

I worry about suggesting medicating when a bird is potentially very ill, which is why I asked about her weight. If her normal weight was 5-6 pounds, and now she's only 3, I'm just not comfortable suggesting giving her Safeguard for five days. The picture below is the amount of Safeguard that a vet suggested when de-worming a fairly healthy bird.

How much weight has she lost since seeing the vet?

The vet didn’t weigh her, so I’m not sure. She definitely feels thinner than normal— her breastbone is much more pronounced.

Not sure if it’s relevant, but she also seems to be slightly off balance. It could just be things shifting in her crop as she moves, but if she didn’t have the impacted crop I’d be guessing coccidiosis.

I’m not crazy about the enema idea, but I saw this post on reddit that made it sound promising.
