splayed legs and slipped tendons

In my last two hatches I've had a significant increase in splayed legs and slipped tendons (none of which I was able to successfully treat :(. ) WHY?! The only change I made was incorporating an egg turner. I've read it could be heat or humidity, but my incubator readings look good (about 99 degrees, between 50-55% humidity). Help, please.
 
Actually unless you have checked it with a calibrated thermometer, you can not know for sure what the temperature is in the incubator. If it is even a degree or two off it can make a big difference in your hatch.

So if I check with that thermometer and it's off, then what? Adjust according to the thermometer?
