sudden lethargy, rooster unwell...what is wrong??? please help!

The main diet is rice but we give them plenty of vegetables and table scraps. We throw in egg shells for the hen and he frequently eats some also. He's about 5 years old...

So you've never provided them with a nutritionally complete feed? A complete, balanced feed is important if you want them to live a long and healthy life. Nutrient deficiencies can be very serious. Rice, veggies, and scraps will leave significant holes in their nutrition. There are lots of trace minerals that are needed, otherwise there can be serious health complications.
 
We fed them store bought feed until they were 6 months old. What can I add to supplement their diet?

Right now, they typically get rice in the morning, then vegetables at night + leftovers every once in a while. Our hen is an egg pecker so the two of them get about half the eggs she lays. They free range at least twice a week, and it's been raining a lot lately so they've been eating lots of slugs. Our hen is doing fantastic, but Freckles is the one having the problems.

Egg eating can actually be an indicator of protein deficiency. Get them on a proper feed that is formulated for chickens. Unmedicated chick starter, grower, all flock, or flock raiser are all fairly accessible.
