Third chick died after hatching

My broody hen successfully hatched 2 chicks at day 21. They are doing well. I had 4 eggs left and decided to leave them under her a couple extra days.. It is day 23 and one hatched early this morning and I found it dead outside the nest. Another just hatched,I found it wet and laying on its back. It was still moving alittle so I left it with mother hen. I noticed some blood on the egg shell. (a tiny bit) I wonder if this one is going to die too. The first hatches were active quite quickly.
 
Sounds like it fell out the nest and died of cold. Where was the one lying on its back? It needs to be under the broody to have any chance.
Edited to add that a little blood in the shell is no cause for concern.

So three are dead? Any alive and still okay?
Is mama still on the nest? Was baby kicked out of the nest?

Yes the first two are alive and doing very well. The third to hatch died and was kicked out of the nest. The 4th is now doing well also after I warmed it up. So three alive and well. The mother hen is taking care of them. There are 2 eggs left to hatch but its been 25 days so I wonder if I should give up on them.

The one laying on its back is doing well now. It had just hatched and the mom was busy with the other two chicks so I put it in a bin with a light and warmed it up and it started chirping in about 5 minutes. I then put it under the mom later. It is doing very well now.
