Tips needed for incubating chicken eggs

I have incubated a batch of 40 chicken eggs, all different breeds, and have had only 4 hatch! They all needed assistance too! I candled all of the eggs, and they were fertile. I am planning to hatch a second batch, and I need some tips so that more than 4 hatch this time.
