Troubles hatching out of Membrane?

What do you call the white film that is sort of like the second layer to the shell?
Is that the Membrane?
If that is what you call it I’m having so issues with my hens that are hatching their own chicks.
All have had some chicks I have found dead under them that have completely remover or particularly removed the shell of the egg, but still have the Membrane around them.
There was one last night that was alive, but in the membrane. I put it back under mama as had only been a few hours, I feel she knows best, and I’ve read if you intervene too much the chick won’t make it. Checked this morning and the poor chick was dead in the membrane.
Why is this happening?
Always unfortunate when eggs do not hatch, but I hate when these little guys have come so far and die at this stage.
I try not to interfere as I feel the hen and Mother Nature know best, but maybe I should be?
What is causing this?

Is there any chance that other hens are accessing the broody hen's nest and inadvertently causing her to crush the eggs. I have only every had this problem with extremely large/clumsy hens.

Not that I think. With the one last night definitely not as she’s on a dog kennel while they hatch.
