Unwell chicken

My chicken has been going back and forth on weight, and was starting to act kind of lethargic which was extremely out of character. I decided to separate her from the rest of the flock.

Chicken info:
Breed: Buff Orpington/Golden Sexlink Mix
Age: Unknown (guessing 3 -5)
Weight: normally 5, but lately 4lbs

Symptoms:
Sleeping a lot
Not eating
Drinking for long periods of time
Watery green/yellow stool/foamy/black spot
Needs help regurgitating when she jerks her head

I cannot figure out for the life of me what it could be or even what to do to help her. Seeing her this way makes me want to cry.
 
What have you treated her for in the past if anything?

And is it very hot in your area?
I haven't treated her for anything, all I did was change from her regular feed to fermenting it and she got a lot better. It was 100°F on Saturday, but she seemed fine. Today was a lot cooler so I got concerned that sudden change.

What do you mean "needs help regurgitating when she jerks her head"?
Are you vomiting her?

Drinking a lot of water sounds like a crop issue. I'd check her crop first thing in the morning to see if it's emptying. If it's not, then begin treating the crop https://www.backyardchickens.com/ar...w-to-know-which-one-youre-dealing-with.73607/

No mention of when her last egg was. She may be having some reproductive difficulties which can be common in laying hens. Cancer, EYP, Salpingitis, etc.

When hens begin having issues, then the crop can begin to slow as well.
I'd work on the crop, consider deworming or getting a fecal float. Do provide her with food/water and grit.

I've been helping to vomit her. I just checked this morning and her crop was still full and very squishy. When I vomit her she's been vomit black solider fly larvae and scoby that I've given her a couple weeks ago. She finally started eating this morning and she attempted to eat some grit last night around 20:00.
I know I've dewormed her in April and is supposed to do it again on October. I think I'll look into a fecal float and see what happens.
I have no clue if sugar water would be good for her, but she seems to perk up after I give it to her.
 
If the crop is sour, then the sugar could increase yeast growth.

I'd treat for Sour Crop.
Vomiting a chicken is very risky, they can aspirate fluid or even choke and die.

Do make food and water available.
