You will want to inspect her looking through the feathers for crawling bug which will mostly be Lice. To look for mites, go out at night, inspect the birds around the vent area and rub a white paper towel all around the roosting bars to see if it comes clean or if you have bugs or smears of red or reddish brown (smooshed mites) on your towel.
 
It's good that you have treated for parasites.

Unfortunately, laying hens can be prone to having reproductive disorders - Cancer, EYP, Internal Laying, etc. So the sickest one may be suffering from one of these conditions, it's always hard to know unless you lose them and have a necropsy through your state lab which will give you a report. You can investigate further on your own looking at internals to see if you find anything obvious, a lot of folks do that.


Often the best you can do is treat symptoms as they arise. Address parasites, keep the hens fed and watered, etc.
I wanted to come back and update, I think the elector PSP has restored my flock to normal health. They are beginning to molt now so I am seeing a reduction in egg production from that, but I do see feather regrowth in bald spots, and redness/irritation is disappearing. I’m no longer observing scratching. My hen that was on major decline has started to regain weight and I see her take the feed. I had her in a hen hospital for 2 weeks with barely any pooping, observed watery loose stools with urates.. she is now pooping again. It isn’t quite back to normal poop but it is definitely progress. Her comb is red again too, it was pale. She is no longer lethargic, she is acting like a chicken again. No egg from her again yet but I’m hoping soon. Her last egg was in May. Current poo attached (sidewalk was just now, roost was last night). I’m feeling very encouraged!!!
