What to write on a 4-H Poultry Record

It has 2 Questions I need help on. First it asks about management practices (vaccination, medication, caponizing). It also asks about poultry income. It has 10 different slots for those questions. Should I write N/A, Nothing, or something else.

You need to fill in your practices, not ignore the questions. If you don't vaccinate, say "none"; if you do, say what and probably when. etc. for the other categories.

With income you will need to list both your expenses and any income (sales, prize money, etc.) You should be asking your 4H leader so you know what your local expectations are.
