When to Feed Layer Feed and Placement of Nesting Boxes

2 boxes should be fine.

I personally don't switch to layer until they actually start laying, but many people feed it when they're "close enough" (18 weeks+) so up to you and what's convenient for you.

As far as nest placement, I'd say make it more convenient for yourself. Just keep them lower than roosts. Chickens aren't too picky about where the boxes are. Though my personal preference is also to have them at least a couple feet from the pop door, so any muddy feet get "wiped off" as they walk across the bedding.
 
Our 6 chickens will be 17 weeks old next week so I want to start getting them ready for the possibility of laying eggs.
I open/install nests around 15 weeks.
Two minimum. I had 5 Golden Comets in my first Flock and two nests. I've seen a hen waiting for a nest many times.

I've seen two in a nest a couple of times.
If you have high production types, I would put in three. But you can always add another later.
I offer Oyster Shells in a separate container after 16 weeks.
If all are the same breed/type of chicken, I would switch to layer feed when you get the first egg.
If you have different breeds I would wait till one of each is laying.
I feed a Non-Medicated Starter-Grower 18% or a All-Flock crumble 20% to my hens, 22 months old, with a separate container of Oyster Shells. GC
