Why are my chicks pipping and dying?

I seem to get a high percentage of chicks that pip and then die. Five out of fifteen in the last batch.
 
Improper humidity is the typical cause. Temp too high can cause it too because they have difficulty finishing yolk absorption. Are they pipping early? That would be the sign of temps too high.

They actually pipped one or two days late this time. For the last three days I had the humidity set at 70%, although it never got that high. It stabilized in the low to mid 60's. The incubator is an R-com 20.
