It is not one of those pimply teenager, it is for fully grown adults.

I think tweakers do it because they actually feel bugs crawling on their skin.

They had something like a cut that scabbed over, they would just constantly pick at it.

It was uneasy watching them, sitting there watching CNN at the hospital waiting room. both of them were just sitting there, doing it for 30 minutes picking on one wound, using every fingernail they got to pick a scab. It is so disgusting and I wonder if there is a mental reason for it or plain nervousness like someone who would jiggle their lower legs under the chair for hours.
