Why?

I have this hen, shes about 3 or 4 years old and she had finished molting about a month and a half ago, started laying again as normal, then she stopped less then 2 weeks later. Any reason why?

None of my 3+ year old hens lay anymore. Maybe it depends on the breed and where you live? But she may have given all the eggs she had.

My older American Dominique hens (3+) seem to require more sunlight and warmth to keep them in lay. That means late spring and early summer and eggs not every day from a given hen. Same hens still produce on average the largest eggs.

Well, maybe. Although I heard mixed breeds usually lay more eggs, and she's far from pure.

Time will tell. I hope your hen has a few more years of egg laying in her. All I can tell you is none of my 3+ year old hens give eggs anymore. But I have heard other people say that some hens can lay for many years. I hope you get lucky.
