Worms, listlessness, and problems walking

One of my hens has been having problems walking. I've separated her and confined her to a crate with bedding, food, and water. I've soaked her legs in Epsom salt and water. I started giving her super Vitamin B complex. Today I noticed a messy butt so started soaking her...yikes! Tons of tiny white worms emerged. What do I do now?

Thank you! Just soaked her again, trimmed feathers and applied Vetericyn. She's so lethargic and pitiful.
