Worms?

Her stance (see photo below) I would say she is having reproductive problems. Have you felt inside the vent about 1-2" just to make sure there's no egg stuck?
Feel the abdomen for bloat/swelling or feeling of fluid. Egg Yolk Peritonitis, cancer, tumors, Salpingitis and Internal Laying are fairly common in laying hens - she may be suffering from on of those conditions.

How much rice does she eat a day? It's not that nutritional, so I would probably eliminate that. Offer her normal feed (layer feed?) along with the tuna or some egg. I would also give her some poultry vitamins for a couple of days.

She has better energy today. Her comb has a healthier color. She is eating some layer feed by herself. I just give her a little rice and mostly tuna/egg/layer feed mix, she likes it. She walks around sometimes, but sits down most of the time.
Her abdomen is always fat, but she has been losing weight because hasn't been eating enough, I am not sure those are fat or fluid. It's squishy but I can feel her breathing as I put a little pressure on it. I try to massage her abdomen and she seems to like it. I don't feel any egg inside her vent.

She is not walking as fast as before, and her feet are slightly twisted inward.

I give her tuna/layer feed mix, she is crazy about it.

She does look like she's feeling better.
This can happen with hens that are having problems - they can have good and bad days.
Often all you can do is keep watch on them, routinely check the crop to make sure it's empty in the morning, look them over for lice/mites every couple of weeks and see how it goes.
 
My hen has been dropping watery poop for last 2 weeks. She doesn't walk around too much and losses appetite for chicken feeds and rice, but drinks a lot of water and still goes crazy for meal worm treats . Some spots of her comb also appear to be darker than usual.

She is about 4 years old, and has stopped laying eggs since 5 months ago. I have attached pics of her poop. Not sure she has worms or egg bound.

Is she overweight? Is she allowed to free range and get exercise? Do you feed her alot of food scraps and carbohydrates such as bread, rice or pasta?

I am from bay area, California.

Today she didn't drink much water. Her crop is empty in the morning. She always has a fat abdomen, so I can't really tell if it is bloat/swelling.

Today I gave her cooked rice and she seemed to like it and ate quite a lot (I used to give her uncooked rice for treat).

After eating she went to a shade spot resting

I gave her warm bath, she seemed to get worse after that, but she was still eating rice.

After eating she just kept standing sometimes closed her eyes, it probably was a bad idea to give her a warm bath.

I thought so too, wouldn’t that cause the bloat? That’s why they stopped throwing rice at weddings, someone told me it would swell in the birds and pretty much rupture their insides. I’m new to keeping chickens so I didn’t want to say anything, since it may not be the same with chickens.

Yes, pretty much the same for chickens.

She sounds like she is having issues with her crop, maybe just feed her layer feed for a while without too many treats or no treats at all for a bit.
 
Update: She is doing better today. Walk round more often,eat more layer feed. Her poop looks healthier. I gave her pearl parley water yesterday, hope that helps.
Thanks for everyone's advice. They are free range. I have been giving them uncooked rice for treat for last 3+ years, I should stop doing that now and only give her layer feed.
This one always has a fat belly like 2X size of the other one, and a big comb and high temper, fights like a rooster. She had laid 2 eggs/day for several times.
