Yellowish pasty poop and slugish

I have a Rhode island hen that is acting a little slow and not feeling well. She pecks the ground put does not really eat as much as the rest of the hens. Her poop is very sticky and yellowish in color. What kind of supplements should I start giving? She gets multi vitamin water with probiotics, Purina layer feed, scratch grains mix. She is maybe starting to loose a little weight also.
 
How old is she? Chickens can get coccidiosis as chicks up to 20 weeks, but also can get new strains when older from new birds or by moving to a new yard. Corid is the treatment for cocci. Worms can cause diarrhea also, and is best treated by worming with Valbazen or SafeGuard liquid goat wormer. She should be getting enough supplements with vitamins and probiotics--you can probably decrease those to 3 days a week, but make sure you aren't giving electrolytes with the vitamins.
 
Thanks for the info.She is 3 years old. I did put some new chicks in about a year ago. They were about 4 months old and had been eating medicated food.
