That looks exactly like what happened to my roo when I had an outbreak of bumblefoot. It traveled up his leg and came out his "hock" in a wound that looked similar to that. Poor baby.
He lost the use of the joint in that leg and had to hop around. :(

After researching vent gleet, I am certain she has had it off and on. I have treated her with molasses before for a different issue, and it cleared up so I will have to look into that. Perhaps it helped the gleet? She was given to us and has always had it off and on.

I agree with Sharon, your setup is good and if the chick was in the middle, well who knows? I am sorry about your chick, I think that is the hardest thing when one dies and you just don't know why!
