It is just layers and perspective. Try it. It is not hard, just a little time consuming getting rid of the back ground around the subject in the picture they are stepping out of.
 
I use photoshop 5 and photoshop Elements. I think Elements is easier to use to do this type of thing though. All the tools are laid out in front of you and you don't have to go looking for them.
 
Imagine that you have a picture of a flower and another picture of a butterfly, and another picture of a green meadow. If you cut out the flower and the butterfly and place them the right way on the meadow, it would look like you have a butterfly on a flower in a meadow. The flower, butterfly and meadow are different layers.
