14 week old polish pullet eats only when I wake her!

Hi I have a 14 week old Polish pullet named Ducky that I bought recently with 2 others of the same age and breed and 4 other 1 month old polish pullets. When I bought them they all seemed fine and dandy but I did notice the day after I bought them home Ducky didn't stay with the others that came with her and she was a little bit lighter. I figured she maybe was stressed from the transport. Anyway fast forward about a week or so later and I got to let them out and Ducky is laying in the pullet house in the corner, I got to grab her and she runs out but shortly later lays down again. I immediately take her in the house and put her in a small dog crate with some heat I get electrolytes in to her and feed her some chicken food mush. She perks up and is walking around I continue to give her extra attention and keep her separated and warm. When I bought her in I noticed she is all bony no meat is on her! Then I noticed her beard and head puff are very puffy and realize she may not be able to see at all. I fix that problem but the next day she has no energy at all except when I wake her up to eat she pecks like a crazy fiend like 4 times then falls asleep again this has been going on for 2 days now she will eat and drink when I wake her for a very short time maybe 20-30 seconds then fall right back to sleep.
1) What type of bird , age and weight (does the chicken seem or feel lighter or thinner than the others.) She is a Polish pullet, about 14 weeks, very very thin.

2) What is the behavior, exactly. She can not stay awake long enough to eat or drink unless I wake her again and again.

3) How long has the bird been exhibiting symptoms? 2 Days

4) Are other birds exhibiting the same symptoms? No they are all very healthy and hyper.

5) Is there any bleeding, injury, broken bones or other sign of trauma. No

6) What happened, if anything that you know of, that may have caused the situation. Maybe she's been starving because she couldn't see??

7) What has the bird been eating and drinking, if at all. Cooked eggs, applesauce mixed into the eggs sometimes, Gatorade, water.

8) How does the poop look? Normal? Bloody? Runny? etc. Runny @ first now it looks much better.

9) What has been the treatment you have administered so far? Warmth and making sure she is eating.

10 ) What is your intent as far as treatment? For example, do you want to treat completely yourself, or do you need help in stabilizing the bird til you can get to a vet? There are not any vets that will take a chicken in.

11) If you have a picture of the wound or condition, please post it. It may help. Posted below.

12) Describe the housing/bedding in use
She was in with the other chicks I bought in a quarantine pen with a small run and pine shavings in the coop.

Polish chickens may be kept from food and water or bullied easily. She also could be suffering from possible coccidiosis. I would make sure to feed her individually a couple of times a day, and may sure that she is drinking. Chopped egg, tuna, or liver can be good treats. Getting them outside to walk around and peck at things may help as well, in case she is low in pecking order or afraid. Trim her feathers around her eyes.
 
Separate her from the other chickens. Get some Poultry NutriDrench and give her a couple of drops, or put some other electrolytes in her water and offer them to her several times an hour. Feel of her crop to check if it is empty of full, firm, soft, or puffy. You may have to bring her in to give her fluids today and try to get her eating. Was she also vaccinated for Mareks disease? I hope you can save her.
 
Yes she is vaccinated for Marecks I have her in and isolated I brought her in as soon as I saw she was not feeling well. Her crop fills then empties just fine. I wake her a lot to eat and drink and when she does eat and drink she is enthusiastic about it but it only lasts 30 seconds max.
