6 week old hen with prolapsed uterus.

I had a 6 week old barred rock hen with what seems to be a prolapse uterus (she was pooping fine). We had put it back in but it came out again after trying to poop. She was in so much pain so my parents thought it was in her best interest if we put her out of her misery. Has anyone else had this issue? Was it preventable?
 
It sounds like she was constipated, but there could have been something else going on. When they suffer a prolapse, apply either honey or hydrocortisone cream to the tissue, and try to push back in, then hold it for a few minutes. Make sure the brooder temperature is not too hot, since that can cause dehydration and constipation. If a chick is constipated, a few drops of molasses can be given in some wet food to work as a laxative. Sorry for your loss.
