No, I'm sorry, I can't. I don't have any right now and I can't find an image of it online either.
Here's an image I was able to find. It shows directions to make a stock solution. 30mL = 2 Tablespoons.

I have a confession to make...

The liquid dose was easy to figure out, but the powder dose made my head spin and it took me a long time to check, double check and find all the info on it.

That says 3/4 teaspoon, but it's the 0.012%, which is fine for cattle... poultry should get the 0.024% (1.5 teaspoons).
