Any idea what this could be or causing it! I'm puzzled. (pic)

Similar to OP's problem, I believe. Instead of yanking feathers out totally, a bird will strip the center bare. In parrots it is usually connected to stress or nutritional issues.

It's happening in places she can't pick (like around her comb), and it's gradual. She also shakes her head like something is bothering her - but no visible bugs and dusting doesn't seem to help.

I have found that sometimes the seven isn't strong enough. I had a similar issue switched to permitherin dust and sprayed the coop with it as well.....I think Northern fowl mites live in the coop and only come out at night to feed then go back into the cracks. It looks like she is scratching herself. Just based on my own experience...Good luck!

It comes in liquid concentrate. I use 1 oz per gallon in a garden sprayer and soak the coop during the day so it's dry by night...roost, laying boxes, walls everything is wood in my coop!
It also comes in a dust. Tractor supply has it. It's called poultry dust in the poultry section but if you go to the garden section they have a larger dusting can of the same stuff. I even put the dust under my shavings in the laying box. I think it runs around $6. I use two cans to dust my 90 birds once a month. sometimes I fall behind but I try to stay on top of it! Be sure to use a mask or a respirator when apply the dust to your birds. You'll have to shower after it for sure!!!!
