our rabbit came down with twisted neck last week. Vet diagnosed parasite cause and he was put on medication.

Have had a slight improvement, he can now stand away from wall without falling over and spinning to right himself but neck is still very much twisted and he couldn't be put back in his cage how he is. He is eating well but last couple of days I've started having trouble getting water into him (have to use dropper as worried he will hurt himself on metal end of water bottle when he flips)

I am wondering if he is ever likely to recover to a decent quality of life or if this is as good as it will get.

Rabbits with wry neck can recover; IME, it depends on how bad the case was. I've had some that always had a bit of a tilt, and some that if you didn't know they'd had it, you sure couldn't tell by looking. "Eating well" is a hopeful sign. Is he continuing to regain control? Sometimes it takes weeks for them to get to whatever level of recovery they are going to achieve.

It will be two weeks on next Monday. He got to stand still without wall support after a week but no improvement since.
