Baby dying before hatched

I really need help. I am using incubator with egg turning. I don't know why baby ducks are dying in the last stage of incubation. when I open the them up, some are dead inside the protective skin and some were able to push through the the protective skin but still inside the shell and some others were able to crack the shell but couldn't get out.
 
Im a newbie hatcher but with the knowledge iv read been told off people on this forum it sort of sounds like your humidity maybe wasnt high enough. But that may not be the issue though sounds like a humidity problem.
Hopefully someone with more insight can help you.

It could be a # of things. It could be bacteria in your incubator. It could be humidity (too high or too low). Do they look really wet inside (like too much liquid) or too dry (the membrane is white and shrunk around the chick)?
