I’ve tried draining her with a needle and then again with a syringe, but nothing came out and it just started bleeding (I thought it was ascitis). However, whenever we feel around the area, it almost feels like another sack is inside of her. Any help would be appreciated greatly!!

Is she still laying eggs?

The sack you are feeling inside of her may be from a reproductive disorder like Internal Laying, Salpingitis or Egg Yolk Peritonitis.
If she's eating/drinking and relatively active, then I would just monitor her if she's more pet than livestock. A hen may live for quite a while with a bulge like that.
